Position Overview

The Case Manager assists clients who require social service support and works with individuals to identify their needs and goals. The position interviews and evaluates clients and develops Independent Living Plans in collaboration with them. The Case Manager identifies and connects clients with appropriate community resources and assists with relocating clients to permanent housing. The role requires strong communication, organization, cultural sensitivity, and a commitment to supporting individuals and families experiencing complex challenges.

 

This position is located in Staten Island, New York. 

Key Responsibilities

  • Build professional relationships with clients and maintain confidentiality.
  • Review client eligibility documents and explore non-shelter alternatives.
  • Create and manage client files and records.
  • Conduct intake interviews and psycho-social assessments.
  • Develop and update Independent Living Plans with short- and long-term goals.
  • Refer clients to permanent housing and community-based services.
  • Support clients in overcoming barriers and advocating for themselves.
  • Track client progress and document updates through regular contact.
  • Follow up with clients and referral agencies to monitor service outcomes.
  • Provide data for required reports (weekly, monthly, quarterly, annual).
  • Act as a liaison with outside organizations for education, healthcare, childcare, housing, and legal matters.

Requirements

  • High School Diploma or equivalent required; Associate or Bachelor’s degree in human services, Social Work, or a related field strongly preferred.
  • At least 1 year of experience in case management or social services working with individuals experiencing homelessness, substance use, mental health challenges, or other complex needs.
  • Strong interpersonal skills and ability to build trust with diverse individuals.
  • Familiarity with New York City housing, healthcare, legal, childcare, and educational resources.
  • Strong problem-solving, organizational, and detail-oriented skills.

Pay Range

USD $24.48 - USD $25.03 /Hr.
