The Staten Island Workforce1 Career Center is partnering with Community Agency for Senior Citizens (CASC) to find qualified candidates for a NY Connects Outreach/Resource Specialist in Staten Island, NY. Community Agency for Senior Citizens (CASC) is a nonprofit organization dedicated to helping Staten Islanders age 60 and older live as independently as possible in their communities. Through the NY Connects program, CASC provides objective information, assistance, and referrals to individuals of all ages with long-term care needs, helping them access services and supports that promote independence and well-being.

Position Overview

The NY Connects Outreach/Resource Specialist provides information, referrals, and assistance to individuals, caregivers, and professionals seeking long-term care services and supports. This role conducts client screenings, assists with public benefits applications, provides person-centered counseling, and connects individuals with appropriate resources. The Outreach/Resource Specialist also participates in community outreach activities, maintains accurate documentation, and collaborates with community partners to support individuals with accessing needed services.

 

This position is located in Staten Island, NY.

Key Responsibilities

  • Provide information and referrals on long-term care services and supports.
  • Conduct No Wrong Door screenings by phone and in person.
  • Assist with Medicaid, SNAP, HEAP, and other public benefits applications.
  • Conduct home visits to assist with benefits and entitlements.
  • Provide person-centered counseling and care transition assistance.
  • Maintain accurate client records and required documentation.
  • Participate in community outreach events and presentations.
  • Follow NYC Department for the Aging program standards and complete required trainings.

Requirements

  • Bachelor's degree (BSW, BS, or BA) required; Master of Social Work preferred.
  • At least 1 year of experience in health, human, or social services serving older adults, individuals with developmental or physical disabilities, behavioral health, and/or substance use populations required.
  • Must have a Valid driver's license and personal vehicle for work-related travel.
  • Familiarity with Medicare, Social Security, and Human Resources Administration (HRA).
  • Experience conducting community outreach and tabling at events.
  • Proficiency in Microsoft Word, Excel, and Outlook.
  • Bilingual in English and Spanish preferred.
  • Ability to work occasional evenings and weekends.

Pay Range

USD $22.59 - USD $23.55 /Hr.
