The Staten Island Workforce1 Career Center is partnering with the Community Agency for Senior Citizens (CASC) to find qualified candidates for a Case Manager in Staten Island, New York. Community Agency for Senior Citizens (CASC) is a nonprofit organization dedicated to helping older adults remain safely and independently in their homes and communities. Through case management and supportive services, CASC connects seniors with resources that promote health, independence, and quality of life.

Position Overview

The Case Manager provides case management services to adults age 60 and older by assessing client needs, developing individualized care plans, and coordinating supportive services. This role works closely with clients, families, and community partners to connect older adults with appropriate resources and benefits. The Case Manager also maintains accurate documentation, monitors service delivery, and advocates for clients to help them remain safely in their homes and communities.

 

This position is located in Staten Island, NY.

Key Responsibilities

  • Conduct assessments for program clients.
  • Develop individualized care plans based on client needs and preferences.
  • Authorize eligible home care, housekeeping, and home-delivered meal services with program supervisor approval.
  • Establish community service linkages and make appropriate referrals.
  • Provide benefits and entitlement counseling and assist with applications and follow-up.
  • Advocate for clients and support their service needs.
  • Maintain accurate case records and required statistical documentation.
  • Assist with intake activities, assigned program calls, and other program activities.

Requirements

  • Bachelor's degree (BSW, BS, or BA) required.
  • Minimum of 2 years of case management experience.
  • Proficiency in Microsoft Word and Excel required.
  • Experience using the Department of the Aging VIVE database preferred.
  • Experience providing direct services to older adults preferred.
  • Detail-oriented, organized, self-motivated, and strong problem-solving skills.
  • Ability to manage time effectively, meet deadlines, and work under pressure.
  • Fluency in a second language is a plus.

Pay Range

USD $24.03 - USD $26.44 /Hr.
