Key Responsibilities
Guide families through a structured, three-phase treatment model: Initial, Baseline, and Stabilization.
Conduct comprehensive assessments and develop individualized service plans.
Coordinate multidisciplinary interventions tailored to each family's unique needs.
Build meaningful relationships with families over an average case duration of 12 months.
Collaborate with internal and external providers to ensure safety, stability, and well-being.
Requirements
BSW or bachelor's degree in human services REQUIRED.
Some knowledge of the child welfare system.
Fluency in English and Spanish a Plus
Citywide travel required- 14 cases up to 4 visits with each family per month
