Position Title: Case Manager
Classification: Full-time
Compensation Range: $48,000-50,000
Position Summary: Reporting to the Assistant Director of Case Management, the Case Manager is responsible for developing and monitoring comprehensive participant service plans in collaboration with assigned Wellness and VRS track participants. This includes, but not necessarily limited to, internal and external referrals for services to stabilize medical and mental health conditions, barrier removal, job readiness coaching, job placement assistance, and retention services.
