Position Overview

The Direct Service Provider/Life Skills Coach will assist members of Person-Centered Care Services (PCCS) to achieve a high-quality lifestyle in accordance with NADSP Code of Ethics and Core Competencies and through meaningful activities and relationships and serve as a teacher, leader, mentor, and friend.

Key Responsibilities

  • To provide proper supervision and ensure the safety and well-being of the individual at all times.
  • To have the person receiving supports’ best interest in mind and focus on a person-centered approach.
  • Focus on valued outcomes that the individual would like to achieve to help improve the quality of life.
  • Assist person with activities of daily living. 
  • Access the community with the person as needed.
  • Attend all staff meeting and training sessions as required. 
  • Keep the supervisory staff informed of all issues related to the service. 
  • Report and document all incidents that may occur with the individual within 24 hours.
  • Implement and follow the person’s authorized plan and goals.
  • Maintain and document accurate records and data of the person’s progress in accordance with the frequency noted. 
  • Submit all documentation on time and in an appropriate fashion. 
  • Discuss with supervisor updates needed to plan as they arise.
  • Complete all mandatory trainings by state and agency requirements.
  • Complete clock in and out of payroll system and employee health record.
  • Use of vehicle and/or public transportation with person receiving supports. 
  • Perform any other duties deemed necessary assigned by supervisor.

Pay Range

USD $18.00 - USD $25.00 /Hr.
